Media Consent and Release Form

Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Tourism HR Canada and its affiliates and agents to use my image and likeness and/or any interview statements from me in its publications, advertising, or other media activities (including the internet and social media).

This consent includes, but is not limited to:
• permission to interview, film, photograph, tape, or otherwise make a video reproduction of me and/or record my voice;
• permission to use my name, as well as my place of work and title/position/role therein at the time of consent; and
• permission to use quotes from any interview (or excerpts of such quotes), any film, photograph(s), tape(s), or reproduction(s) of me, and/or any recording of my voice, in part or in whole, in its publications, in newspapers, magazines, and other print media, on television, radio, and electronic media (including the internet and social media), in theatrical media, and/or in mailings for education and awareness.

I authorize the presence of such media representatives and technicians as necessary to the process. I acknowledge Tourism HR Canada cannot be held responsible for the information I provide, for the contents of any stories and/or photographs or film taken, or the conduct of media representatives.

I waive rights to privacy and compensation, which I may have in connection with such use of my name and likeness, including rights to written copy that may be created in connection with video production, editing, and promotion therewith. I hereby release Tourism HR Canada and its employees, agents, and sub-contractors from any or all claims, actions, and liability for damages, losses, or expenses of any sort which may arise in connection with such interviews and/or recordings and their use.
